The Nigerian Safety Investigation Bureau (NSIB) has announced that it has initiated an investigation into a serious incident involving a Fly Bird HS 125 aircraft, registered as 5NKAL.
The aircraft was operating a flight from Nnamdi Azikiwe International Airport, Abuja (DNAA), to Kotoka International Airport, Accra (DGAA), when it experienced technical difficulties on December 6, 2024.
Preliminary information indicates that the aircraft, carrying four individuals (three crew members and one passenger), was climbing through flight level 240 en route to flight level 280 when the crew reported an issue with engine number 2. At approximately 17:54 UTC, while positioned about 25 nautical miles southwest of the VOR station VONUK, the crew requested to divert back to Abuja due to the engine indication. The request was promptly granted, and the aircraft was cleared to descend to flight level 190.
During the descent, the aircraft lost engine number 2 at flight level 230. The crew decided to return to Abuja. Upon approaching Nnamdi Azikiwe International Airport at around 5,000 feet, the crew declared a Mayday, reporting the loss of engine number 1. The crew successfully landed the aircraft at Abuja Airport at 18:16 UTC. Fortunately, there were no injuries reported, and all individuals on board are safe.
In a statement, Mrs. Bimbo Oladeji, Director of Public Affairs and Family Assistance at the Nigerian Safety Investigation Bureau, noted that NSIB has launched an investigation to determine the circumstances surrounding the engine malfunction and the subsequent loss of both engines. The investigation team is gathering evidence, conducting interviews, and reviewing the aircraft’s flight data and maintenance records to uncover the underlying causes. The goal of this investigation is to prevent future occurrences and enhance aviation safety.
